If you require CE contact hour verification from a previous conference, all requests for certificates must be made in writing to:

American Nurses Association, PO Box 504410, St. Louis, MO 63150-4410.

Include a check for $10.00 with your request. Please include: street address and phone number, your e-mail address, the title of each event you attended, and the date and time of the events. Contact hour certificates will be e-mailed to the address provided.
